In order to help expedite your visit to the Counseling Center we have provided four basic Walk-In Clinic intake forms that can be reviewed, downloaded, printed and completed prior to coming to the Center. (You will still need to enter your responses into the Center's data entry stations when you come to Walk-In.) In addition, there is an assessment that must be filled out in the Center.
